Hexes generally are not that big of a problem. If you get something like backfire or diversion on you, just have a monk remove it (backfire, not diversion) or just not cast for x seconds. If you get spammed with hexes, laugh at the enemy mesmer or necro who just wasted his whole energy bar hexing you.
Against melee, the best defense is kiting them. You'd be surprised at how many warriors don't take sprint. There are a ton of good anti-melee skills as well, pretty much every class has a few.
As for conditions, they are not really a problem. Almost every monk has mend ailment in their skill slot. It becomes a problem when the other team consistently spreads conditions to your whole team, but even then if they are not doing direct damage on top of that it is easily countered.
